    • Submit at least 2 completed recommendations – one from a teacher, or dramatic coach.
    • Island Community Chorus – Peter Boak Music Recommendation form
    • Describe why this Major/Course opportunity is important to you.
  • If you are a descendant who worked at Bagehot Backs, list details.
  • Blankenship/Hughes Applicants Essay – not to exceed 2 pages, answering “How can I help improve the quality of life on the Vineyard?”
  • Personal Statement – Tell us a little about yourself.

  • Are you presently enrolled, or are you applying to a post secondary school?
  • Provide name of current school (if applicable), and intended major/field of study.
  • Name of school you are applying to, and intended major/field of study.
  • Specify tuition type, 2 or 4 years, or vocational
  • Specify degree sought, Bachelor, Associate, Certificate, etc.
  • Upload a PDF of your transcript listing all coursework to date. Name and student ID must be on them. PDF only, No links are accepted.

  • Describe your carrier goals.
  • Explain any unique circumstances that may adversely affect you.
  • Counselor Recommendation
    •  MVRHS Seniors – Upload page 3 of the common application from your Guidance Office.
  • Recommendation
    • All applicants are required to submit 1 letter of recommendation with their application.

  • Provide financial information for you and your parent/guardian.
  • Provide your EFC (Expected Financial Contribution) to your education.
  • List past awards received form the Endowment.
  • List total amount of all awards received this year.

  • Provide links to music or videos showcasing talent.
  • Upload supporting files showing the above.
  • Recommendations – submit 2 completed recommendation forms, one from music teacher, or dramatic coach.
